2025-2026 Financial Aid Forms

Please allow 7-10 business days for processing the following forms. 

• Dependency Override Request Form

This request is for unusual circumstances in which you cannot provide parental information on the FASFA®.
 

• Financial Aid Revision Form 

This request is to make any adjustments to your student financial aid.
 

• SAP Appeal Form

This request is for students not meeting satisfactory academic progress. Please note: request should be submitted prior to the start of the semester in which the student is requesting an appeal. 
 

• Special Circumstances Appeal

This request is for if you or your family have experienced significant changes in income or other financial situations not reflected on your FAFSA, you may request a review through a Special Circumstances process. This allows our office to consider adjustments to your financial aid eligibility based on documented changes such as job loss, medical expenses, or other unexpected events.
 

• Summer Aid Request Form

In order to receive any financial aid for the summer semester, a student must submit this request. Please note: according to the RPCC Academic Calendar, Academic Years begin with the summer semester leading in to the fall semester.
 

• Total and Permanent Disability Discharge Verification Form

The Total and Permanent Disability Discharge Verification should be completed if the National Student Loan Data System (NSLDS) indicates that you have had one or more prior student loans discharged due to total and permanent disability.
 

• VA Education Enrollment Form

This form is used to certify that you will be using your benefits for the coming semester. Please note: this form must be completed prior to the start of each semester you plan to use your benefits. 
 

• VA Responsibility Form

This form is used to acknowledge student responsibility to maintain eligibility for VA benefits. Please note: this form must be completed at the prior to the beginning of each Academic Year.
